Working collaboratively alongside healthcare professionals, families, carers, and community services to support each person’s individual care needs and overall wellbeing.
At North Dublin Home Care, we recognise that person-centred support works best when people, families, and professionals work together in partnership. Every individual’s needs, circumstances, and goals are unique, and effective communication and collaboration help ensure support is coordinated, responsive, and tailored to the person receiving care.
Navigating the wide range of health, social care, and community supports available can often feel complex and overwhelming for individuals and families, particularly during periods of illness, hospital discharge, changing care needs, or times of uncertainty. With over 50 years of experience supporting people within the community, we are well established within the sector and have developed strong working relationships with many multidisciplinary teams and community support services across our catchment areas.
Our Home Support Workers and management teams work closely, where appropriate, with:
- Public Health Nurses (PHNs)
- General Practitioners (GPs)
- Occupational Therapists
- Physiotherapists
- Speech and Language Therapists
- Dietitians
- Social Workers
- Mental Health Services
- Hospital discharge teams
- Family carers and advocates
- Community and voluntary support organisations
These established partnerships help support continuity of care and allow us to respond appropriately to changing needs, routines, health conditions, or support requirements over time.
They also help us support service users and families in accessing relevant services, information, and professional supports when needed.

While our role focuses on home support and assistance with daily living, we can also help identify concerns or changes in wellbeing and communicate these, where appropriate, to family members or relevant healthcare professionals. This collaborative approach helps ensure people receive the right support, at the right time, while remaining safe and supported within their own homes and communities.
We understand that care and support are about more than individual services alone. They are about relationships, communication, trust, and ensuring people feel supported, informed, and listened to throughout their journey of care.
At the centre of all partnerships is the person receiving support. We believe individuals should be actively involved in decisions about their care and empowered to express their preferences, goals, and wishes. By working together, we can help promote independence, dignity, wellbeing, and the best possible quality of life.
